Eligibility Requirements
Must be a U.S. resident at least 21 years of age
Must be pursuing fertility treatment (IVF, IUI, surrogacy, adoption, etc.)
Must have a Gift of Parenthood account
Must provide documentation of treatment plan from a licensed provider
One application per person per quarter
